Bring all old records. This should include labs, consultants
reports and reports of any radiological studies.

Bring all medications, vitamins and supplements.
|

Do not bring actual x-rays only reports.
|
|
|
Allow us enough time for your visit. You may be here 2 ½ to 3
hours. Please arrive at the time you have been given.

|
Bring all your insurance information. This allows us to submit
your claim so that you will be reimbursed for your visit. |
